Bromley Learning Disability Partnership Board 1.Healthy Weight – increase number of people who have healthy weight, reduce risk of illness due to not having healthy weight. LD nurse attends planning groups The Big Health Plan

Bromley Learning Disability Partnership Board 2. Health checks and black books – everyone who wants black book has one, everyone who wants a health check has at least one, one per year even better. Getting GP’s to join schemes. ( Part of Self Assessment Framework too)

Bromley Learning Disability Partnership Board 3. PCT Self Assessment (Big Health Check up) – More green ratings, No red ratings. Has own action plan led by Nicola Symes (Claire Lewin employed to help)

Bromley Learning Disability Partnership Board 4 meetings a year usually. Bromley healthcare have joined. Next meeting April – review PCTSelf Assessment work and review big health plan.
