Prevention and Independence update Public Health 1
Update Overview of mapping of current provision Emerging model for prevention Key workstreams – Prevention & Independence
Mapping of existing provision Mapping undertaken of prevention services – primary, secondary, tertiary Both statutory services and external provision Gaps in information (VCS, external providers) Provides snapshot overview of provision and does not reflect quality or performance
Mapping conclusions Difficult to pinpoint specific ‘gaps’ due to the complex nature of individual cases Does not consider demand management implications / capacity issues Mapping needs to inform development of the prevention model and be used to develop a complete directory of services of current prevention services Pathways between social care and the range of services in existence need to be strengthened
Emerging model for prevention Rationale High level of deprivation across the town and the variation in health inequalities between wards Growing austerity measures impacting on service provision, often leading to deterioration of poor health and the subsequent demand management issue Significant work within public health around remodelling preventative services has taken place over recent months, including the decommissioning of some services Evidence suggests that individuals often present with multiple issues, therefore there is a greater need for integrated services
