Script Your Future Adherence Challenge University of Pittsburgh 2016 A Campaign of the National Consumers League

What is “Script Your Future?”  National campaign to raise awareness about the importance of medication adherence  Provide tools for patients and health care professionals  Hold annual medication adherence challenge competition

Why Medication Adherence?

Medication Adherence Challenge Month long annual outreach competition held in January-March Interprofessional student teams required Focus on medication adherence among patients affected by diabetes, respiratory diseases, cardiovascular disease and smoking 5 Schools/Colleges recognized nationally

What is in your folder? Materials for You – A “How-To” instructional sheet for conducting a SYF patient encounter – Sheet for you to record information from the patient encounter to transfer to PITT Form – Photography guidelines and photo release form – “Ten tips” for medication adherence sheet – Advertising flyer to hang up at your site – Smoking cessation notepad to write notes on for patients Materials for Patients – 10 copies of the following: Patient Pledges Wallet cards Online resources sheet Prescription sheets **The wallet cards are available in 10 different languages online **See Julie during the week to pick up additional record sheets and patient materials

SYF Patient Encounter Step by Step Step 1: Introduction Step 2: MMAS-4 Survey Step 3: Questions and Interventions Step 4: Pledge of Adherence Step 5: Complete Pitt Forms for Each SYF encounter

Step 1: Introduction Ways to engage the patient – “I would like to talk to you about how you take your medications. It will only take 3-5 minutes.” – “Would you like to have your blood pressure taken today?” Times when this interaction can occur – During counseling sessions – During the time patient is waiting for their meds/dropping of a prescription – Any patient who is near the pharmacy – After taking the patient’s blood pressure

Step 2: MMAS – 4 Survey Goal of Survey: Gain a baseline understanding of the patient’s adherence “Let’s start with a few questions to get a baseline understanding of your adherence.” Ask the 4 Questions, then place a check mark on the record sheet for each “Yes” received Scoring the patient: Count number of checks (“Yes” = 1 Point, “No” = 0 Points) Inform the patient of their adherence classification using the table on “How-To” sheet

Step 3: Questions and Interventions Series of questions to ask the patient and interventions to improve their adherence Question 1: What is your age? – Action: Record this on the “record” sheet Question 2: “What medical conditions do you currently have?” – Action: Record condition on “record” sheet as either CV, Diabetes, Respiratory, Smoking or Other – Intervention #1: Go over the “Prescription” sheet with the patient – Intervention #2: Ask to take patient’s BP if not done in the beginning

Step 3: Questions and Interventions Question 3: “How many medications are you currently taking?” – Action: Record the exact number on “Record” Sheet Question 4: How often do you miss a dose? What do you do if you miss a dose? – Intervention: Go over the “online resources” sheet – emphasize text reminders

Step 3: Questions and Interventions Question 5: “How do you currently remember to take your medications?” – Intervention: Hand wallet card to patient and fill out medications (time permitting) Question 6: Do you have any other questions about your medications? – Answer any questions the patient has – Consult preceptor if unable to answer

Step 4: Pledge to Medication Adherence “Would you mind filling out this pledge to take your medications?” Explain pledge and provide an example if necessary Have patient fill out pledge and collect it – Everyone will turn their pledges into Jessica Cole and Karen Quach each Monday after site visits Mark on record sheet if the patient filled out a pledge of medication adherence At end of encounter, check services provided to patient on record sheet

Step 5: PITT Form SYF Survey has been incorporated into the PITT Form Click “Yes” to question: Was this a SYF encounter? Use the record sheet to transfer the information from the patient encounters to the PITT Form Click “Experiential Learning 4 Community Pharmacy Site” for the last question Please save paper forms in your folder until end of competition!

Potential Hurdles and How to Work Around Them Not being able to set up a table out front – Solution #1: See if the pharmacy has a counseling room and if that room can be used to conduct SYF encounters – Solution #2: Set up a table in the break room and conduct SYF encounters with the employees at the store Call your district coach if there are any problems

Key Points to Keep In Mind Currently ten sets of the materials per student to hand to patients (not unlimited supply) – BOTTOM LINE: If you do not completely fill out the record sheet for a patient do not hand them a packet of sheets Do NOT falsify information Overall Goal For Each Person: Minimum 10 SYF Patient Encounters over the course of 3 days at site If two people at a site, they need to decide which of the two the patient encounter belongs to. Only one person documents each patient encounter.

Pictures at Sites Take at least 1 picture of the SYF project at your site and send to: Photos used to show our progress throughout the campaign Rules regarding taking pictures at sites – See photo guidelines handout and release forms in your folder! – Contact Elizabeth Garcia if you have further questions.
