Nature and Scope of Service:  The goal of home care is to keep residents in their apartments as long as safely possible, while maintaining quality of life and optimum health.  Home Health Aide activities would include:  Medication Administration ~ Medication Reminder  Assistance with ADL’s ~ dressing, bathing  Simple wound care  Application of TEDS/Lower Extremity Wraps  Ambulation

Nature and Scope of Service :  ALC (Assisted Living) offers assistance with activities of daily living, as well as quality medical services, while maintaining the highest level of independence safely possible.  Supportive Care residents have increased needs and require assistance and monitoring of activities of daily living.  Increased staff/resident ratio.  Home Health Aide services would include:  Assisting with ADL’s ~ dressing, bathing, feeding  Assisting with mobility  Assisting with medications  Leading meaningful group activities

Nature and Scope of Service:  The Short Term Rehabilitation offers short term rehabilitation care including physical, occupational and speech therapy services in an effort to prepare residents for their return to home.  CNA responsibilities include:  Assistance with ADL’s ~ dressing, grooming  Assistance with mobility  Assistance with TEDS/Lower Extremity wraps

 CNA Certification  High School graduate or GED

 FTE and Supplemental positions  8 hour shifts  ALC ~ Supportive Care ~ STRC  6 am to 2:30 pm  2:00 pm to 10:30 pm  10:00 pm to 6:30 am  Home Health  7 am to 3:30 pm  3 pm to 10:30 pm  Salary Range  $11.91 to $16.90
