Disease Prevention/Health Promotion Service Implementation must clearly provide the following six elements: 1. Tier of each program you will provide 2. The name of each program 3. A description of each program 4. Qualifications of the instructors or practitioners

Disease Prevention/Health Promotion Service Implementation 5. If minimal tier, especially without a name, additional detail: a. Program type (for example fitness education, blood pressure screening, etc) b. Health conditions the program helps c. Program documentation or references d. Credentials or certification of the instructor or practitioner
