Disease Prevention/Health Promotion Service Implementation must clearly provide the following six elements: 1. Tier of each program you will provide 2. The name of each program 3. A description of each program 4. Qualifications of the instructors or practitioners

Disease Prevention/Health Promotion Service Implementation 5. If minimal tier, especially without a name, additional detail: a. Program type (for example fitness education, blood pressure screening, etc) b. Health conditions the program helps c. Program documentation or references d. Credentials or certification of the instructor or practitioner

Disease Prevention/Health Promotion Service Implementation: 6. Other information to give a good picture of the service you are implementing: a. Who is overseeing (AAA staff, contractor) b. Locations where programs are held (meal sites, community centers, senior centers, churches, schools, etc.) c. Partners, other community organizations d. Any other descriptive information
