OHSAA Officials Ejection Report Filing an Officials Report Officials shall file a report with the school and the OHSAA office whenever a coach or player is ejected from an athletic contest. Use an Official's Report form which can be found by clicking the link below. An official WILL complete the form online. Please use one form per offending school. Each offending school will need its own form completed. The report shall be filed with the OHSAA within 48 hours of the ejection. Whenever an ejection occurs, the ejecting official shall speak with the offender's principal/athletic director no later than the first school day following the ejection. An official failing to follow the ejection protocol and/or failing to file the Official's Report may be penalized in accordance with Section VIII.A.5 of the OHSAA Handbook for Officials.

OHSAA Officials Ejection Report The "Official's Report" form is also used to report good or poor sportsmanship, severe injuries, facility problems, or equipment problems. Ejections other then players or coaches and other items of which the Commissioner should be made aware must be provided. The reporting official will receive notification of resolution upon receipt of the reported school's reply. To complete your Ejection Form, click on the link below and you will be redirected to officials portal to log in. Use your MyOHSAA username and password to enter the site. Usernames and passwords are CASE SENSITIVE. Once inside your account, click the Official Tab near the upper right corner of the screen. Next, select the Submit New Ejection Form option along the left side of the page. Enter the details of the report and submit your report electronically. Please select the User Guide option on the left side of the page for more details on how to complete the Ejection Form.

OHSAA Officials Ejection Report Some users will need to verify their address before they can complete the Ejection Form. To do this, log into your account from the officials portal at Next, click the link to verify your . Follow the instructions on the new page to complete the verification process. After you've verified your address, you can complete the Ejection Form process as outlined above.

OHSAA Officials Ejection Report Ejection Report User Guide- Official You must submit Reports using the online form provided here. All fields must be entered as you complete the form. 1. Select the Sport, Gender and Level of the contest for which you are submitting the report 2. Click the Event Date field to pull up a calendar- choose the date which the event took place 3. Click the Event Time drop down menu- choose the time which the event took place 4. Enter the name of the location of the event 5. Click the “Select a school” option for the Offending School a. Begin typing the name of the school: a list of schools will appear for you to select in a matter of seconds- choose the appropriate Offending school b. The name of the school, Athletic Director, and AD address will appear for you once a school has been selected

Begin typing the name of the school: a list of schools will appear for you to select in a matter of seconds- choose the appropriate school b. Repeat this process to add more than one Opponent School (if necessary) 7. Click the “Add an official” option to select additional members of your Officiating crew a. Begin typing the name of the official: a list of officials will appear for you to select in a matter of seconds- choose the appropriate official b. Repeat as many times as necessary to completely identify members of your crew 8. Click the “Add a person” option to select the person you are reporting a. Enter the first and last name of the individual b. Select the coach, player, or other option 9. Select the type of report that you wish to submit a. Ejection b. Equipment c. Sportsmanship d. Injury e. Facility

OHSAA Officials Ejection Report 10. Enter specific details about the issue. Please be as thorough as necessary in this official report 11. Use the Submit Report option to electronically send your Report to OHSAA and the school a. Once you’ve submitted the report, this process is complete 12. In the event of an Ejection, make sure that you contact the appropriate Principal or Athletic Director at the school to advise them of the situation.

OHSAA Officials Ejection Report Required Reports Required reports include ejected contestants, ejected coaches, and incidences of crowd disturbance, disqualifications or undesirable behavior. (Soccer and Ice Hockey disqualifications and Wrestling disqualifications due to flagrant misconduct are critical reports to be filed immediately). Officials shall file a report with the school and the OHSAA office whenever a coach or player is ejected from an athletic contest. Requested Reports Requested reports include examples of especially noteworthy sportsmanship (good or bad), severe injuries, substandard facilities, equipment or uniform issues, or any concern for which there is need for immediate attention. The Ejection Form is also used to report these issues. Ejection Protocol Whenever an official is required to eject an athlete or coach from a contest, the official must follow the protocol outlined here. 1. Within the rules of the sport, announce the ejection by making eye contact with the ejected player or coach. Do not make physical contact with the individual. Eye contact can be made from a distance of twenty feet, but should not require an official to be closer than six to ten feet from the individual.

OHSAA Officials Ejection Report 2. Request the player to go immediately to the bench and to the head coach. 3. One game official will go to the sidelines, make eye contact with the head coach and announce that the player (identify by number or lane or some accepted method) has been ejected from this contest and state the reason. 4. Unless players create a disturbance, they may remain on the bench. 5. The ejection of a coach requires that the official approach the coach, announce the ejection to the coach and request that the coach leave the competition area. In addition, the official must ask which coach or school representative will function as the head coach in the ejected coach’s absence. 6. Officials are reminded that only assistant coaches or school representatives may act in place of the coach. 7. The ejected coach shall leave the competition area and go to the locker room (indoor sports) or the bus or other area. An ejected coach my not be allowed to become a spectator. 8. Following the contest, an official shall contact the Athletic Director or Principal of the offender’s school to advise the administrator of the ejection. Contact may be made after the game if an administrator is present or by phone in the morning on the first school day after the contest.

OHSAA Officials Ejection Report 9. Finally, one official shall file a report of the ejection using the SUBMIT NEW EJECTION FORM option at officials.myohsaa.org. Please indicate the ejected person’s first and last name. 10. A separate report must be filed for each individual ejection. 11. The Athletic Director will respond to the report by submitting the School Response form (sent via ) to OHSAA. The school may include a summary of the action taken. 12. Upon receipt of the Athletic Director’s reply, an announcing resolution will be sent to all parties involved. NOTE: It is extremely important that officials follow the ejection protocol. Failure to follow ejection protocol may result in a maximum fine of $100 to the official, a penalty in accordance with Section VIII.A.5 of the OHSAA Handbook for Officials
