> Competing the Application Log In Personnel at LEAs that have a web portal login: Go to the Education website and login: Click on Login in the left column. Select PASES in the drop down box at the bottom of the page and hit Go. 2

> Competing the Application Log In Personnel who do not have a web portal login:  Go to the Education website and login: supplemental_education_services/7416/home  Near the top of the page you will click on the link for “Provider Application”. 3

> Agency Information  Please provide current contact information including address.  Contact should remain the same and will require clearances 4

> Applicant Info  SES only  Application was also for EAP in the past  EAP has been discontinued  LEA, Charter School, for profit, etc. 5

> Service Area  Refer to the building list to determine if a particular building is required to offer SES.  PSSA data is not yet available for the school year so this list is subject to change  Select  By district  By county (All districts in county)  Statewide  All SES eligible Districts in State – only has to be chose for one county to get entire State  NOTE: Not providing services to an area listed on the application may result in a penalty in the next year’s application 6

> 1. How will the provider ensure that the selected evidence-based tutoring models are aligned with the school curriculum and with Pennsylvania’s reading, math and science standards? (Maximum length is 3000 characters) (Rubric 8.1)  Describe connection/alignment between your program and PA State standards.  Cite specific standards.  All content area standards can be accessed at under the Clear Standards tab. 8

> 2. Explain the method of communication between the provider and the classroom teacher and how often student progress reports will be made available to the classroom teacher. (Maximum length is 3000 characters) (Rubric 8.2) Method of communication; progress reports Describe the connection between your instructional program and the program in place at the student’s school. > 3. Explain the method of communication between the provider and the parents and how often student progress reports will be provided for the parent. > 4. What assessments are used to determine student eligibility and benchmarks? How will tutors know that students are making progress within the tutoring program session so they can adjust instruction and provide feedback to teachers and parents? (Maximum length is 3000 characters) (Rubric 8.4) How will the student be assessed and prescribed an Individual Learning Plan? 11

> 5. Where will the tutoring services be offered? Please indicate whether services will be offered at a school site or a different location. List all locations including name of site, address and phone number. [Note: Tutoring is not permitted in the student's or provider's home]. (Maximum length is 3000 characters) (Rubric 8.5) Address the following: – How will services be provided? – How will services be accessed by students? – Transportation – Addresses of Tutoring Locations 12

> 6. What are the educational or instructional accommodations for eligible students with disabilities? Will language assistance for eligible Limited English Proficiency (LEP/ELL) students be provided? If so which languages (i.e. Spanish, Chinese) will be offered? Explain qualifications and experience. > 8. Please provide a brief abstract that outlines your program. School districts will have the ability to include this when mailing out parent notification letters and will also be publicly viewable on the Department of Education’s website. (Maximum length is 1500 characters) PDE may modify or return this narrative to the provider for edit/modification if it is deemed that information contained is inaccurate or misleading. 14

> 3. Anticipated duration of program. Maximum hourly rate = $50/hour for 3 or fewer students per tutor $45/hour for 4 to 5 students per tutor 4. List your maximum student:tutor ratios. The maximum group size for tutoring is 5 students per instructor. 16

> Evidence Based Tutoring Models  (Rubric 8.6) Please describe your Evidence-Based research Model. In the second column describe objectives and strategies used in the model.  How will student needs be identified?  Are all tutors trained in the model?  What remediation methods will be utilized?  What formative assessments are used? 17

> Evidence Based Tutoring Models (cont’)  Description of strategies must assure the reviewer that student needs are being met. Note: Homework help, such as working on subjects assigned by the classroom teacher, is not sufficient to meet the requirements of this program. Choose the subjects (reading/math/science) for which tutoring will be offered.  Discuss how the Five Essential Elements of learning to read are addressed: Phonemic Awareness, Fluency, Vocabulary Building and Comprehension.  Provide citations from research to justify the instructional methodology. 18

> 4. Please provide a brief statement regarding the qualifications of your tutors. What professional development do you provide for your instructors? Please include your Professional Development Plan. Describe how you will ensure that all tutoring is aligned with classroom practice. (Maximum length is 3000 characters) 22

> Program Evaluation & Accountability (Rubric 8.1) 1. Explain past effectiveness in increasing student achievement in reading/math/science. Preferably include achievement data with valid and reliable assessment results from a state, district or an independent test developer. Applicant may include measure of improvement using school grades or may list percentages of students improving one year or more as a result of the listed program. Data must be the result of the individual applicant, not a national organization. If the applicant is a new tutor, state the professional development in the model selected. (Maximum length is 3000 characters) 23

> Financial Statements Must be submitted once every two years. Acceptable documents include: annual financial statements, fiscal audits, Dunn & Bradstreet reports, etc. If you are a new business, you must include your business plan. 24

> Assurances There are two you must submit. Must be signed in BLUE ink. Application cannot be reviewed until all documents are received. 25

> Budget (Rubric 8.9) Projected overall budget and organizational/accounting structure: 1.Please provide a description of the organization, including information such as projected overall budget, length of time in operation, number of existing locations, number of staff, organizational structure, etc. (Maximum length is 3000 characters) 2.Please provide a description of the organization’s history of providing similar supplemental educational services, including the number of previous contracts, the diversity of clients, the number of students served, etc. (Maximum length is 3000 characters) 26

> Budget (Rubric 8.9) (cont’) Projected overall budget and organizational/accounting structure: 3.Please provide the names, titles, addresses, phone numbers, and addresses (if available) of at least three references who contracted for similar services within the past two years. (Maximum length is 3000 characters) 4.Attach copies of the organization’s tax returns for the past two years or other evidence of fiscal soundness e.g. annual financial statements, fiscal audits, Dunn & Bradstreet reports, etc., submitted as Appendices. Only required once every two (2) years. (Maximum length is 3000 characters) 27

> Tutors Tutor names are entered into this section by PDE as they are approved (original clearances have been received by PDE). All tutors previously entered will be deleted before the application is scored. If you have tutors that have had continuous employment with your company since they have submitted original clearances to PDE, you can submit a letter stating this, listing their names and signed by the CEO or Superintendent in lieu of new clearances. Proof of this employment must be available upon request. 28

> Tutors Tutors must be on the state approved list before being in front of the children. Violation of this can result in termination of the provider’s approved status. 29

> Documents to be Submitted Before Application can be Reviewed: Signed Assurances Contact’s Child Abuse Clearance Contact’s Criminal Background Check Contact’s FBI Clearances (Cogent PA only) Verification of Business Status Financial Statements Note: if documents are not received by 4:00 PM on April 26, 2013, the application will not be accepted. 30

> Other Requirements: Attendance at Mandatory Meeting – this is in addition to meetings that districts may require Rubric Requirements Met Requirements for Progress Met 31

> Application will be Reviewed After: Online application is submitted by the deadline – April 12, All required documents are received by the deadline – April 26, Submission in a timely manner allows for feedback from IU 4 regarding missing items. If you wait until the last minute to submit, we are unable to notify you of missing pieces for submission by the deadline. 32
