APPLICATION RECORD INCLUDE THE FOLLOWING: 1. Applicator’s complete name, license and certification number. 2. Customer’s name and address. 3. Treated site (trees, shrubs, etc.). 4. Specific location of application site. 5. Month, day, year, approximate start and finish times of the application. 6. Brand, product or common chemical name AND EPA product registration number of each pesticide used. 7. Each location, other than a licensed business location, at which the pesticide was mixed and loaded. 8. At least one of the following: a. The concentration and total quantity of each pesticide used. OR b. The amount of each pesticide applied per unit area and the total area treated.
