WHY DO WE NEED TO INVESTIGATE ACCIDENTS & INCIDENTS?

UK Key annual figures 2012/2013 148 workers killed at work 78 000 other injuries to employees were reported under RIDDOR 27 million working days were lost due to work-related illness and workplace injury (2011/12) 175 000 over-7-day absence injuries occurred (LFS) Workplace injuries and ill-health (excluding cancer) cost society an estimated £13.8 billion in 2010/11 1.1 million working people were suffering from a work-related illness (2011/12)

ACCIDENT INCIDENT RATE (AIR) The incidence rate is based on the number of accidents, taken over a fixed period, per 100,000 employees. Incidence rate= Number of reported injuries in a year x 100,000 Average number of employees in a year Scenario: During a 12-month period in our window company there were six reportable accidents and during that year the company employed an average of 120 employees, what is the incidence rate?

Why do we investigate accidents? 1. To collect the information you need to pass on to the enforcing authority (HSE) 2. To identify the cause of the accident and/or incident to stop it happening again. 3. To get information needed for an insurance claim. 4. To find out the cost of an accident.

Key benefits of accident investigation are to: 1. Prevent more accidents & incidents. 2. Make a safer work environment. 3. Prevent more business losses from disruption, down-time and lost business 4. Improve workers’ morale.

An accident investigation: FOLLOWING A METHODICAL PROCESS OF STEPS: STEP 1: Preserving the scene. The accident location is photographed to record the area. STEP 2: A statement is taken from the person who had the accident. STEP 3: The accident report form is filled in and sent off if it is reportable under RIDDOR. STEP 4: Witnesses are interviewed to provide further evidence as the cause of the incident. STEP 5: An analysis of the accident is undertaken to establish the primary cause. STEP 6: New control measures are devised to establish if the existing system of working can be revised. STEP 7: Any changes to the system of working or control measures are then implemented.

Activity 2: “Manuel has no luck today!” You have to create a short story about how Manuel suffers an accident and develop a further investigation using the 5 first steps of the process. Within your group, write down your story and investigation on the paper provided. You could use the format of your choice: Cartoon or bubble drawings, bullet points, story board or just in words, but remember that you will also have to fill in the HSE form provided. To complete steps 6 and 7, exchange the investigation with the other group and develop adequate control measures for the other team’s scenario.
