30/05/ Prevalence Estimates suggest 3,099 people living with dementia in Herefordshire Population of Herefordshire – 178,000 Current prevalence via GP Qof data – 35% which represents circa 1,090 people with confirmed diagnosis known to service Target for 2014/15 – 1,358 people with confirmed diagnosis of dementia known to service

3 Community Dementia Service Community and Primary Care facing Able to provide early diagnosis and support Able to provide support, advice and education across organisational boundaries Facilitate social integration and living well with dementia Include those with learning disabilities Provide support to Primary care and neighbourhood teams Coordinate support following crisis or hospital admission, working with key partners

4 Functions of Community Dementia Service Community Dementia Nurses Principle function is to provide ongoing support from diagnosis to end of life Provide proactive intervention to reduce escalation of need and to minimise crisis or admission to hospital To focus on facilitating supportive interagency and ‘community asset’ working with support for primary care, to include district nursing and GP’s

5 Care Home In-reach Service To support care providers with the management of individuals with dementia, in particular challenging behavior, so the person can be maintained in their care setting for as long as possible Training for care home staff

6 Specialist Memory Clinic Work to the standards of the Memory Clinic National Accreditation Programme to provide rapid specialist assessment and diagnosis of suspected dementia based on three appointment model. Assessment Diagnosis Initial support (including psychology, education and signposting.

Dementia Link Workers Commissioned by 2gether as a partnership with the Alzheimer's Society. Provide advice, support to service users and families Bring vast national and local expertise to the service Use their established service and links to community assets.
