2 Progress on Personalisation Single point of contact for social care. First point of contact seeks to direct people to appropriate services. For those requiring social care assistance an initial assessment made. Following this targeted interventions may be required. These interventions have the goal of enablement/reablement They may include services or equipment.

3 Progress on Personalisation People requiring longer term support have supported self assessment.(“indicative amount”) If required this is developed into a Support Plan with the person. The Support Plan is the basis of Self Directed support. The Support Plan is reviewed and updated. This is work is done through the local Integrated Teams. (Apart from Mental Health and Learning Disability).

4 Self Directed Support As a care managed package “brokered” by the CHSC teams. As a Direct Payment (SUGGEST) A payment to another party (for instance a Trust) An Individual Service Fund managed through a provider. A fund managed by a “broker”.

5 Progress on Service Review –Services providing advice, guidance and information to the general or specific populations –Services providing specific things which may be required by people as part of their support plan as part of Self Directed Support. –Established current funding sources and commissioners

6 Service Review Process To be able to commission services from the Third Sector that fit the overall personalisation strategy. Reviewing against the Strategic Priorities set out in commissioning plans. Reviewed against the objectives and outcomes of the Contract or Service Level Agreement. The process designed to particularly focus on strategic relevance.

7 Review Process Completion of Template Questionnaire completed by Stakeholders Consideration by the Integrated Commissioning Team using the Criteria. Develop a overall view of what needs to be commissioned and draw up new Service Specifications New Service Specifications tendered.

8 Criteria Consistent with Commissioning intentions Current Performance Partnership Working Responsiveness to Individual Need Accessibility (Equality Groups) Involvement of service users and carers Management and Organisation Value for Money Stakeholder Feedback
