A portfolio should reflect the best that you have to offer.

Cover Sheet This will be the first impression of you! Includes the following items Your name Class name Date Title (you create your own title)

Table of Contents You need to have a table of contents You will not be able to add the actual page numbers to the items until the very end of this class. You need to keep your work samples all year

Purpose This is to be a brief description of your portfolio and the purpose of your portfolio

Letter of Introduction This section is to tell about you. Introduce yourself Why you are writing this letter? Interest areas Career expectations Explain how you could be of a benefit

Cover Letter Use business letter format Introduce yourself Prospective position you are applying for Goes on top of your resume

Resume Compact summary of your life Your information (address, etc.) Your education Your experiences Your skills Your extra curricular activities Your references

Training and Skills All related healthcare classes Seminars Workshops Certifications Healthcare skills you have mastered

Work Samples You must include at least two work samples from your diagnostic services related activities. Written descriptions with each work sample

Success Samples You must include at least two examples of successes you have had in diagnostic services.

Digital Images You must include at least ten digital images that portrays you in work or class scenarios in which you are actively involved in healthcare activities. This is a way to demonstrate to other people some of your skills. This is a way to emphasize some of your training. Digital images may be pictures or video clips.

Reflective Essay To show your thoughts on all of your healthcare related courses, seminars, workshops, volunteer and work experiences.

Career Objective What is your career objective? Research and describe the duties of your career objective List educational requirements What are the working conditions? What postsecondary schools offer training or education in your career objective?

Goals You are to list five short term goals. You are to list two long term goals. You are to create a time line to accomplish these goals.

References You must list five references Include the following: Name Address Phone numbers addresses Fax numbers

Resources You must list all resources, books, internet sites, and/or people used in the development of this portfolio.

Portfolio Your portfolio is an ongoing process. It will be a collection of work samples and training session from the entire course.
