Post Stroke Transitional Care Program Physical Activity Programs

Mission Statement ICAN – Independence Centre and Network provides persons with physical disabilities opportunities for independence and community inclusion by providing individualized support and rehabilitation services.

Post Stroke Transitional Program The goals of the program are to maximize stroke recovery and provide multiple avenues for successful community re-integration by:  Offering a comprehensive continuum of services including education, socialization, recreation, assessment and therapy  Connecting individuals to needed community health and social services

Physical Activity Programs T.I.M.E - Together in Movement and Exercise Program TM Physical Activities – for Persons with Physical Disabilities Aqua Fitness – for Persons with Physical Disabilities  Our consulting Physiotherapist and Occupational therapist provide assessment, develop exercise programming, and follow-up for our clients.

T.I.M.E – Together in Movement and Exercise Program TM Exercise 2 times per week for 12 weeks Exercise programs are designed by Physiotherapists (Toronto Rehab and Toronto Parks and Recreation) Exercise programs are led by Rehabilitation Support Workers Exercise circuit begins with seated warm up (5 min) There are 9 stations included in a circuit (40 min) A cool-down is incorporated at the end of the circuit (10 min)

Who is Eligible?  A person with a permanent physical disability, including adults who have had a stroke  16 years of age or older  Have a valid OHIP card  Able to walk 10 metres (about 30 feet) with or without a walking aid  Have a referral form completed by a Physician, PT, OT or RN

Physical Activities The clients are assessed to identify a need for physical activities, such as an upper and lower extremity program. An individualized program is developed to reflect the client’s goals and is reviewed regularly to monitor progress and altered as necessary.

Program Focuses  Flexibility  Muscular Strength and Endurance  Weight control and/or the impact of nutrition on physical fitness  Cardiovascular Conditioning  Recreational Fitness for leisure, social, emotional and spiritual well-being

Who is Eligible?  A person with a permanent physical disability  16 years of age or older  Have a valid OHIP card  Able to have medical/professional needs met by existing community health network  Capable or potentially capable of directing services  Able to be left safely unattended

Aqua Fitness FlexibilityEndurance Muscular Strength Balance Range of Motion Coordination Our program is designed to increase One hour - once a week
