WV monitoring NHSE now collect information on both a quarterly and monthly basis (from July 2014) Quarterly data includes total number of patients, % with a care co-ordinator, numbers with no planned transfer date Monthly figures from CCG’s on no. of discharges from ATU’s and destination Additional data collected in London on new admissions Some issues with accuracy of data collection

What is the current London picture? Quarterly data collection by NHSE March 2014-June 2014 shows slight increase in patients in London 262 patients at March 2014, 269 patients in London at June 2014 However, patients with no planned transfer date reduced from 205 in March to 79 patients by June

What is the current London picture (2) % of patients on a register (91%) and with care co-ordinator (98%) remained similar from March – June 2014 Monthly patient flow figures for August 2014 showed slight decrease in people entering ATU’s; this may be due to inaccurate reporting rather than reality

What progress has been made in London? London Action plan developed Appointment of WV Project Lead for London to lead on the London action plan London work is hosted by London Social Care Partnership and overseen by WV steering group.

Progress in London cont. Learning Disability clinical network set up (linked to NHS and Social Care Outcome Framework). Aim to give Learning Disability a higher profile and clear focus in London Network will have clinical and social care input and will provide expert advice to improve quality of care and experience of receiving services

Progress in London cont. Second Commissioner event held in July 2014 Aim to support the development of collaborative and cross borough commissioning Action plan has been reviewed following the event.

Progress in London cont. Strengthen advocacy – advocacy event held and booklet developed Develop skills of front line workers working on discharge from ATU’s – training for Care Co-ordinators commissioned by NHSE and planned for early November

Market Position statement currently being refreshed with NHS Quarter 1 data Good practice examples have been gathered from London CCGs on supporting discharge from ATU to the community, prevention of admissions and work with young people in transition Aim to share good practice as part of the refresh

Progress in London cont. LSCP and Public Health looking at the development of pan London JSNA to better reflect the complex needs of the WV cohort Would be useful to get examples of any specialist analysis of needs Continuing Health Care Network are looking at national CHC assessment tool to ensure fit for purpose for people with learning disabilities
