Donations There are 2 types of donations… 1. Online Donations = Credit Card donations through your team webpage or your personal webpage 2. Offline Donations = Cash or Checks turned in at Team Info Meetings, Bank Night, or mailed to the ACS Everett Office

Money Envelopes All offline donations need to be placed into an ACS money envelope Turn these in at the Registration Booth before each Team Info Meeting Please be sure to fill out the entire envelope!

General Team Donations Leave the “Team Member Name” & “Team Member Phone Number” Sections Blank We will credit the donation to your TEAM TOTAL

Individual Donations Fill in the “Team Member Name” & “Team Member Phone Number” Sections Blank We will credit the donation to your INDIVIDUAL TOTAL

Offline Donations All offline donations (cash & check) turned in at Team Info Meetings & Relay will be posted online They will show on your webpage thermometer as… “January Fundraising” “Registration Fee” “January Luminaria” Etc…

Recording your Offline Donations AT TEAM INFO MEETINGS: If you would like donor names to scroll on your webpage, you need to… 1. Add the donation to your personal webpage prior to the Team Info Meeting 2. Turn in a completed “Offline Donation Form” inside of your money envelope, so we can confirm the offline donation was received

STEP 1: Adding Offline Donations to your Personal Webpage Click on the “My Progress” Link Click on “Enter Gifts Received Offline” Enter all of the requested information about the Offline Donation and submit the info for processing It will take approximately 1 week for your donation to be confirmed and to show on your webpage

STEP 2: Offline Donation Form Complete the Offline Donation Form Include this form in your Money Envelope when turning in Offline Donations at Team Info Meetings or Relay For Life

BY MAIL: Donations that are mailed to ACS will be automatically recorded and posted to your personal webpage –Must include the a completed Offline Form from your webpage Recording your Offline Donations
