1 How to fill out Fundraising Forms

2 1.Fill out the name of the event-RFL of Kershaw County, SC 2.Fill out the name of your team. 3.Fill out the name of your Team Captain. 4.Fill out the phone number of the Team Captain to contact if there are questions. 5.Fill out the Team captain’s e-mail. 6.Be sure to enter the Team member’s name in the first column if they are to receive credit. (***They must be registered ONLINE in order to get credit so check the site first). If they are not registered or no name is listed in the Team Member Name column then the amount will be credited to the TEAM. We can’t give credit to a team member that isn’t registered. 7.Denote ALL sponsors by filling them out as listed on this sample. (Remember that if a sponsor is listed on the accounting sheet then there should be a sponsor form filled out as well.) 8.Put all luminaria money in the luminaria column. (Remember that if there is luminaria listed on the accounting sheet then there should be a luminaria form filled out as well.) 9.Team fundraising amounts or donations go in the team member donations column. These include bake sales, car washes, feet, cupcakes etc.) 10.Be sure to total all your columns. 11.All columns should add up to your total amount. 12.Your total amount should agree to the total money enclosed in your donation envelope. (CASH+coins+checks) 13.Your accounting sheet TOTAL should agree to your donation envelope total. 14.Sign your accounting sheet at the Team captain Signature line. 15.Keep a copy of this for your records. 16.If there is a discrepancy, we will notify you so that you can correct your records 17.Please fill up the sheet. It’s not necessary to have separate sheets for each item. Let’s not waste the supplies. They cost money too.

3 1.Fill out the Event name at the top. We are Relay For Life of Kershaw Cty, SC. 2.Fill out the date of our event. 3.Fill out the team member name. 4.Fill out the team member’s e-mail. 5.Fill out the phone number. 6.Fill out the team name. (Very important!) 7.Fill out the Team Captain’s name. 8.Enter the amounts from your accounting sheet in the correct column. These amounts should match the accounting sheet. 9.Total amount should agree to the accounting sheet. 10.Have the team captain sign the envelope.

4 1.Fill out the Team Credited & Team member credited at the top of the form so that proper credit is given and we know who to contact if we have questions. 2.Fill out the Business or sponsor name, address, city, state, zip code and telephone. If they have an e-mail or web address please fill this out. ACS needs this information. 3.Fill out the sponsor level by checking a box. 4.Fill out the method of payment. 5.Be sure that they sign the form 6.Last but not least and very important, please make sure that we have the form & payment in hand before the deadline of March 25, 2016.

5 1.Fill out the team name and team member name at the top of the page. This allows us to give proper credit and tells us to contact if we have questions. 2.Write in the amount enclosed 3.Write in the number of luminaria paid for. 4.Fill out In Honor of or In Memory and make sure the names are spelled correctly. 5.Fill out the donor information-name, address, phone and e-mail.
