8 Key concepts CQC registers providers who undertake regulated activities at locations and monitors their compliance with the essential standards at each location Just one form to cover all

9 What is a provider? A provider is the legal entity that is legally responsible for: - The regulated activities that are carried out - Compliance with the essential standards of quality and safety You will need to register as one of the following types of provider: - Partnership - Individual (single-handed GP) - Organisation

10 What are regulated activities? An activity related to care and treatment that requires registration with CQC Most relevant for primary medical services are: - Treatment of disease, disorder or injury - Diagnostic and screening procedures - Surgical procedures - Maternity and midwifery services - Family planning services

11 What is a location? A place from where regulated activities are provided or managed In most cases this will be your practice

17 2. The application process

18 Application overview In July you can set up your online account using your 12 digit code. We will ask you for some basic information. Select your submission window. You can access and amend your application at any time.

19 When completing the form you will need to select: - The regulated activities you undertake - The locations these are managed from - Who manages them at these locations Registered managers have to complete their own section Application details

20 Declare compliance or non-compliance with each of the essential standards. You need to satisfy yourself that you have the evidence you require to declare compliance. You do not need submit additional information with your application If you declare non-compliance – you will need to submit an action plan. Declaration of compliance

21 Registered providers and registered managers must supply either: a GMC number or CQC-countersigned enhanced CRB. We do not require other staff to have CRB for registration GMC number or CRB check

22 Submitting your application We will send you a 28-day statutory notification at the beginning of your submission window. 10-day reminder before your submission window closes. The window system ensures you have the support you need during the registration process. You must submit within your chosen application window.

23 You can delegate some of the form-filling to one other person One person must be responsible for formal sign-off and submission – eg a partner Make the declaration and submit Submitting your application

24 We’ll send you an acknowledgment and confirm the application is being processed We use the information you have provided and consider other information we have received If we have concerns we may ask for more information We will assess your application and we will notify you of our decision After submission
