 Slips, Trips and Falls  Struck by object  Lifting  Climbing  Automobile

 2014 – Slip, trip and fall off premises. Resulted in broken hip claim is on reserve for $185,000. Individual has had 7 previous slips, trips, and fall incidents.  2014 – Slip, trip and fall. Employee fell on ice. Ice removal not the states responsibility at this facility. Employee re-injured an old injury. Total knee replacement. Claim is on reserve at $85,000 will exceed this. Employee had 2 previous slip, trip and fall incidents.
