IN ORDER TO QUALIFY FOR ANY AWARD, COUNCILS MUST BE IN GOOD STANDING WITH SUPREME COUNCIL ASSESSMENT (SCA) AND STATE DEPUTY ASSESSMENT (SDA). FOR MORE DETAILED INFORMATION, PLEASE CONSULT GRAND KNIGHT’S HAND BOOK ON SURGE….WITH SERVICE.

1.Service Personnel Report Form#365 must be completed and returned to the Supreme Council’s Dept. of Fraternal Services by August Council must annually conduct and report at least four (4) major involvement programs in each of the following Service Areas: Church, Community, Council, Family, Youth & Culture of Life. COLUMBIAN AWARD REQUIREMENTS

3. The Annual Survey of Fraternal Activity Report Form#1728 must be completed and returned to the Supreme Council’s Dept. of Fraternal Services by Jan. 31. The completed Columbian Award application must reach the Supreme Council office by June 30. COLUMBIAN AWARD REQUIREMENTS

Any council that is not currently under suspension is eligible and encouraged to compete for this distinguished honor. To participate, the council must complete the State Council Service Program Awards Entry Form #STSP. This form, along with any supporting materials must be sent directly to the state council for the competition. INTERNATIONAL SERVICE PROGRAM AWARDS

SUPREME COUNCIL AWARDS ONE AWARD TO THE COUNCIL IN EACH JURISDICTION THAT SHOWS THE HIGHEST PERCENTAGE INCREASE IN MEMBERSHIP. ONE AWARD TO THE COUNCIL IN EACH JURISDICTION THAT SHOWS THE HIGHEST NUMBER OF MEMBERSHIP ADDITIONS (NET GAIN)

AWARDS  The Star Council Award  The Supreme Council's highest honor, THE STAR COUNCIL AWARD, is awarded to councils who have demonstrated excellence by achieving:  The Fr. McGivney Award for achieving a 7% Net Gain in MEMBERSHIP growth.  The Founders Award for achieving a 2.5% Net Gain in INSURANCE membership growth.  The Columbian Award for conducting and reporting (SP-7) four major involvement programs in each of the SERVICE PROGRAMS Areas.

AWARDS  The Star Council Award  Stars are Made, Not born!  REPORTING - Through timely submission of the Service Program Personnel Report (FORM 365) and the Annual Survey of Fraternal Activity Report (FORM 1728).  Pay SUPREME PER CAPITA by due dates of October 10 and April 10 of each Fraternal Year

AWARDS  Achieve Father McGivney Award  Membership quota: 1.Quota for councils with members is an increase of 7 membership additions over deductions, based on the total active membership of the council as of July 1. 2.Quotas for councils with 101 or more members will be a 7 percent increase of membership additions over deductions, based on total active membership of the council as of July 1.

AWARDS  Achieve Father McGivney Award  Membership quota: 3. A minimum of 30 members is required. (Inactive insurance members are not included when computing the July 1 quota at the Supreme Council office) 4. Additions are credited for new members, juvenile to adult members, reinstatements, reactivations, readmissions and reapplications received, processed and recorded at the Supreme Council office between July 1 and June 30.

Deductions charges against the quota include all suspensions and withdrawals processed and recorded at the Supreme Council office between July 1 and June Net gain or loss is affected only by the forth and fifth items in this section 7.Transfers in or out of the council and deaths are not counted toward or against the quotas. New council quota is based upon the number of members recorded on the notice of Institution. (min 30 )
