Membership Forms O Liability Waiver O Emergency Contact O Player Conduct O Copies available in the Sport Clubs Office O Due on first day of practice OR 9/17, which ever comes first

Team Travel O Travel Forms O Due one week prior to trip O Late Forms: reimbursements are not permitted O Missing Forms: purchases will be canceled (hotel, car rental, etc.) and reimbursements are not permitted O Emergency Contacts O Take with you on all trips
