Where does the Check Sheet fit into the PDSA Cycle?

Why use the Check Sheet in the Classroom Use the Check Sheet to distinguish between opinions and facts Use it to gather data about how often a problem is occurring. Use it to gather data about the type of problem occurring.

Example of a Check Sheet Number of Minutes Spent Studying Math Facts 10 / 810 / 910 / 1010 / 1110 / 1210 / 1510 / 1610 / 1710 / min. 30 min. 25 min. 20 min. 15 min. 10 min. 5 min.

Some Ways to use the Check Sheet Record types of writing errors on student writing samples Excuses for late homework Observations of the weather Items found in a backpack Amount of minutes practiced studying math facts Amount of minutes spent reading each night Amount of time spent completing homework

Things to remember about the Check Sheet The Check Sheet helps to prove or disprove opinions which we may have but may not be accurate. The main purpose of the Check Sheet is to help tabulate the number of occurrences of a given problem or cause. Check Sheets help you break down data into different categories such as causes, problems and etc.
