 Increasing Healthcare costs ◦ Current healthcare system is reactive instead of proactive ◦ Costs continue to increase  Convenience Addressing The Issues… Implementing wellness programs address these areas by offering a way to proactively control healthcare costs – “Bend the Trend”!

 At least one-quarter of the health care costs incurred by working adults are attributed to modifiable health risks such as tobacco use, diet, and lack of exercise.  An average of 28 percent reduction in sick days and 26 percent reduction in health costs.  An average of 30 percent reduction in workers’ compensation and disability management claims.  An average $5.93 to $1 savings-to-cost ratio.

 Five-year overall survival after colon or rectal cancer diagnosis ranges from 93 percent for people treated at the earliest stage to around 8 percent for those whose cancer is diagnosed after it has spread to distant organs (stage IV disease).  Early colonoscopy screening to detect pre-cancerous conditions could reduce the prevalence rate of colon cancer from 60 cases for every 1,000 people to 12 in 1,000.  According to a recent estimate from the American Diabetes Association, project that as many as 1 in 3 American adults will have diabetes in 2050, unless continued education and action is taken to address the issue.

 The CHC Wellness screening is designed to help build awareness and to help identify early warning signs of disease  Anemia, infections & certain kinds of cancers, heart disease and stroke, kidney disease, nutritional and gastrointestinal disorders, liver & gallbladder abnormalities and diabetes  Improve overall health and well being  Lower insurance risk, thus slowing cost increases

 Biometric Screening ◦ A comprehensive 37 test blood draw, blood pressure and height and weight reading performed by one of CHC’s certified phlebotomists.  Health Risk Appraisal (HRA) ◦ Ties in personal medical history with blood work for a complete picture of your health.  Personal Health Report ◦ Available online 2-3 days after the screening ◦ Mailed to your home 2 weeks after the screening (unless you select to Go Green) ◦ Your results mailed to your physician free of charge!

 Yes the screening is completely confidential ◦ Only you will receive a copy of your results ◦ You can elect to have your physician receive a copy ◦ CHC is HIPAA, HITECH & GINA compliant  Your employer will never see your personal screening results  Your health plan will never see your personal screening results

 How do I prepare for the screening? ◦ Fast 10 – 12 hours prior to your appointment.  Is this a drug test? ◦ No, we are only looking at health metrics like cholesterol, blood pressure, glucose, etc.  Can I still see my personal physician? ◦ Yes, we encourage that you bring your results to review with your physician. Bring your results and let them know you have already completed your annual preventive screening.

No out of pocket cost to participate! The screenings will be covered by the preventive care portion of your health plan Is there an incentive for me to participate? YES! If you participate in the health and wellness screening you will entered in a raffle to win a $40 gift card. There will be 5 winners. Most importantly, the biggest incentive is the increased awareness you will have of your health risks, allowing you to seek treatment and prevent the onset of disease.
