From grant vision to implementation Is the plan implementable? Do you have the resources - staff? Have you contacted “Best Practice” models? Have you kept your partners and your community “in the loop”? It probably won’t be a “turn key” process (3-6 months)

Key Word: FLEXIBILITY Stay focused on the work plan and outcomes Work plan adjustments are allowed as long as it meets the desired outcomes – the “feds” are flexible Stay in touch with your project officer Prioritize and re-prioritize – you can’t implement everything at once Prepare early for sustainability – 3 years goes by very quickly!

Data – Data - Data Begin collecting data immediately – you will have to report on your progress annually. More is better Expected outcomes must be measurable – data must match your goals

Evaluation – Performance Review Engage your evaluator immediately Review the HRSA Performance Review process during the first six months of the grant Understanding and anticipating the Performance Review will result in a positive experience Involve your partners in the process

Financial Oversight Track your expenses and review at least monthly Know the requirements and submit HRSA financial reports in a timely manner Keep track of indirects (in-kind)

Monitor Progress Meet regularly with staff to review goals, outcomes, financial parameters Keep partners informed of progress Address challenges and/or barriers when they occur Always evaluate other options

Where are we now? Obtained 2nd round of HRSA funding with expanded services Applying for FQHC new start funding Evaluating other grant opportunities Expanding capacity at the local health department to continue services Continuing to work with local providers
