INDIAN HEALTH BOARD OF MINNEAPOLIS JOB DESCRIPTION HEALTH DISPARITIES SUPERVISOR EMPLOYEE: SUPERVISOR: UPDATED: DEPARTMENT: JOB SUMMARY This position exists to provide oversight of diabetes staff; responsible for the quality implementation of diabetes program activities. ESSENTIAL FUNCTIONS 1.Supervise and coordinate the daily activities of diabetes/diabetes prevention staff. 2.Responsible for staff development, personnel management, scheduling, performance reviews, training and other tasks associated with managing personnel. 3.Annually conduct performance and salary reviews of diabetes/diabetes prevention staff. 4.Facilitate diabetes & diabetes prevention team meetings at least monthly. 5.Attend department and IHB staff meetings, and represent diabetes/diabetes prevention staff at weekly management meetings. 6.Submit monthly department reports as required. 7.Develop and assist in the implementation of the SDPI Diabetes Programs with IHS standards for the programs. 8.Implementation of annual IHS SDPI Diabetes Programs grants reporting. 9.Evaluate, organize and order supplies and educational materials for the SDPI Diabetes Programs. 10.Assist in meeting regulatory standards regarding nutrition and health education of such organizations as JCAHO. ADDITIONAL RESPONSIBILITIES 1.Implement the Lifestyle Balance Curriculum by acting as a Lifestyle Coach as needed. 2.Implement diabetes education through individual counseling/group classes as needed. 3.Provide appropriate support, communication and informational resources to patients. 4.Act as representative of IHB, providing SDPI Diabetes Program materials and resources to community-at-large. 5.Provide health promotion and nutrition education within IHB and in the surrounding community (Twin Cities area). 6.Participate in Performance Improvement activities as needed. 7.Some evenings and weekends will be required. 8.Assume duties assigned by the Medical Director when needed. PLANNING ACTIVITIES 1.Participate in departmental planning efforts in order to meet departmental and organizational goals and maintain standards of care. 2.Maintain knowledge of current and culturally appropriate health care practices and diabetes care and prevention recommendations. 3.Understand current requirements of grant and funding entities. 4.Maintain awareness of IHS productivity requirements. IMPLEMENTATION ACTIVITIES 1.Active involvement in the coordination, planning, implementation and reporting of the SDPI Diabetes Programs grant activities. 2.Assist in implementing the measurable objectives of grants and standards of care in concert with other staff to address the health needs of IHB’s client population and any targeted populations. 3.Provide health care activities in accordance with IHB’s mission statement. 4.Assist in meeting regular standards of BPHC, JCAHO, and HIPAA. KNOWLEDGE, SKILLS AND ABILITIES 1.Professional and personal integrity. 2.Self-disciplined and organized. 3.Aware of culturally appropriate health and diabetes care. 4.Knowledge and respect of the American Indian culture and other culturally diverse populations we serve. 5.Excellent people skills. 6.Excellent written and verbal communication skills. TRAINING AND EDUCATION 1.Minnesota licensure as an RN, Dietitian or equivalent experience. 2.2+years Supervisory experience. 3.Current certification as a Diabetes Educator or willingness to become certified is desirable. 4.2+years experience providing diabetes and related disease health education, especially in an ambulatory setting is desired. 5.Valid Minnesota driver’s license. 6.Computer skills required. Knowledge of Access and Excel databases and desktop publishing useful. 7.Continuing education requirements as needed to maintain license and/or certification. HIPAA IHB makes reasonable efforts to limit access to and use of protected health information (PHI) by employees to the minimum necessary performance of assigned duties as outlined in job descriptions. This position has access restricted to PHI needed to provide treatment and carry out health care operations in assigned clinical area. PHYSICAL DEMANDS An individual in this position may be exposed to:  Adverse indoor and outdoor environmental conditions  Bloodborne pathogens  Physical hazards (e.g. needle pricks, etc.)  Chemical hazards The work requires some physical exertion such as long periods of standing, walking, recurring bending, crouching, stooping, stretching, reaching, or similar activities; The work may require specific, but common, physical characteristics and abilities such as above-average agility and dexterity.
