GENERAL OBJECTIVE  to improve the quality of life  to increase safety, continuity of care and prevention of social isolation  to simplify relationship between citizens and PA, through a social and health network that provides access to patient centered services and support for caregivers  to share data between the different actors involved in the care pathway to a more efficient and effective management of citizen  to minimize errors in health care  to create "knowledge" in a "collaborative way", through statistical processes in the form of open data, supporting the development of new services for the citizen 3 Innovative technology platform to enable services of autonomy and safety to elderly and disabled patient in a Smart Home Environment
