TargetWhat is happening How are we doing on this What is happening in health? The red, amber or green face shows how well we are doing. Most of our targets are on amber Some have moved to green since last year  = 11  = 13 =  = 1  = 20 =4 We have 4 main targets These slides give a summary   Learning Disability Self Assessment

TargetWhat is happening How are we doing on this Target 1  Everyone from Dorset who is living in long stay hospitals (campus) will move into new homes by the end of 2010 Two people from Dorset are living in hospitals out of Dorset There are no people living in hospitals out of Dorset who should have moved on because their treatment has finished  

TargetWhat is happening How are we doing on this When adults or young people leave a hospital (perhaps after being there a long time) they need a plan for what happens next This will be a person centred plan made by the person themselves and all the people who take care of them There will be more people working in community teams to support people who have additional needs 

TargetWhat is happening How are we doing on this Target 2 – overall =  All doctors in Dorset know how many people on their list have a learning disability 267 people have had an annual health check out of 852 in 2009 This year the number will be higher as more doctors surgeries will be doing the health checks. This will include offering people the chance to have health screening (We will know how many by May 2010) 

TargetWhat is happening How are we doing on this Doctors have been asked to use the yellow books to write Health Action Plans. The yellow books are being printed now and will be ready very soon Later this year Health Facilitators will be looking at health action plans to see how good they are and how much they are being used 

TargetWhat is happening How are we doing on this Dorset County Hospital The hospital has a policy that tells staff how they should look after people with a learning disability when they are patients There are 15 hospital champions who will be making sure that all the doctors and nurses know about this. The champions meet every 2 months. They get advice from others and think of ways to make the hospital better for people There is some easy read information on their website, but there should be more  

TargetWhat is happening How are we doing on this Target 3 – overall =  Keeping people safe in NHS care All NHS trusts have a Healthcare for All action plan All health services are getting better at understanding that they need to make changes like - giving people more time - listening to family members - making information easier to understand There is still more work to do and the action plans will be worked on in the Health Action Group Health Action Group

TargetWhat is happening How are we doing on this Health staff are having training in the Mental Capacity Act so they can help people to make decisions about their care and treatment Every 12 months NHS Dorset looks at complaints affecting people with a learning disability If something has gone wrong an action plan is made to make sure it does not happen again 

TargetWhat is happening How are we doing on this Keeping people safe All staff who work with people with a learning disability can have Adult Safeguarding training to make sure they know what to do to keep people safe from abuse 

TargetWhat is happening How are we doing on this Target 4 - overall =  There will be more specialist services in the community Community Learning Disability Teams will have more workers There are two Intensive Support teams in Dorset for people moving out of NHS housing into new homes One is in the East and one is in the West 

TargetWhat is happening How are we doing on this Including people in planning People with learning disabilities and their families attend the partnership board meetings People with learning disabilities sit on interview panels for new staff Health Facilitators talk to people in day services All the actions that people wanted at the health away day have been put into an action plan This work will be looked at by the Health Action Group  Health Action Group

TargetWhat is happening How are we doing on this Older people may need specialist health care People with learning disabilities who have dementia will be able to use the same services as everyone else Community Learning Disability Teams offer checks for people who may have dementia 

TargetWhat is happening How are we doing on this New Horizons A group of people from health and social services meet up to make sure that people with a learning disability can make the best use of mental health services A plan is being written about how mental health services will help all people in Dorset, including people with a learning disability This plan is called 1 in 4 

TargetWhat is happening How are we doing on this People with a learning disability who are in prison need to stay healthy They may need extra help with this Some staff in prisons have had some extra training and more is being planned There is some easy to read information for people who have been arrested 
