The Grad Program Assistant is the one who will fill out the form Identify the Department Chair Identify the College Dean Identify recipients who will review and sign the appointment form
Once the recipients have been identified, an will be sent to the the Grad School Assistant with a secure code to begin competing the form. Copy the access code and resume signing.
Paste the access code and select VALIDATE to begin.

Use the auto-navigation arrow to complete form fields List duties when Member Type “Special” is selected
Attach the CV by selecting attachment icon and choosing file Select finish when form is complete
The form is available to download as a combined PDF
Subsequent recipients will receive notification to view and sign the document
