Periodic Review - changes Periodic Review must take place within 5 years of approval or last periodic review Annual Review takes place alongside periodic review The Chair of the Approval Panel must be drawn from the approved RSO list and training is mandatory for all new Chairs Student meeting with panel The Dean/Associate Dean signs off and submits documentation to RSO 8 weeks prior to the event Feedback from RSO is provided within 2 weeks of receipt and must be reported to Faculty Board or its sub-committees Post Event definitive documentation must be sent electronically to RSO for audit purposes
