Hwy Ops Div2 REPORTING REQIREMENTS FOR FHWA-45, FHWA-810, AND FHWA-47  FHWA-45:  Is required for all projects on the NHS = > $500,000.  Submit two weeks after the award of contract and no later than the fifth of the month after the end of the quarter.  Information on bid prices for major items

Hwy Ops Div3 REPORTING  FHWA-810: Bid tabulation  Is required for all F-A const. project on the NHS.  Submit to Division Administrator.  Certified by STA official.  Show details on 3 low bidders.  Submit within two weeks of the award of contract.

Hwy Ops Div4 REPORTING  FHWA-47:  Is required for all projects on the NHS that are $1 million or more.  Contractor completes at end of contract.  Information on final costs of wages, salaries and materials used by the contractor.  State DOT personnel verifies information.

Hwy Ops Div5 Office of Program Administration 4 UPDATE 4 Donald West retired on 8/3/04, to be replaced by Vince Barone in November. 4 The price trend historical data will be available on web site around March FHWA Form-45/47 study is on hold.

Hwy Ops Div9 USEFUL HINTS  Always report your project in a timely manner.  Make sure all information is completed on the forms.  Include your name and the State in the subject line of the .  Not reporting the data may cause your State to lose funding.  FHWA forms:  The States are to use the new FMIS Improvement type codes, not the work type codes.
