Health and wellbeing in Salford We want our residents across Salford to be healthy and happy in your day-to-day lives How? By assessing the needs of Salford residents to decide the best way to spend money on health and social care

Our priorities for next 3yrs… 1.Ensure all children have the best start in life and develop well during their early years 2.Local residents achieve and maintain a sense of wellbeing by leading a healthy lifestyle supported by resilient communities 3. All local residents can access quality health and social care and use it appropriately

How do we plan to do this? Bring NHS care into the community Integrate care for older people Strengthen community mental health services

High quality –More care closer to/at home and fewer trips to hospital Safe –More senior consultants at specialist hospitals Accessible –Variation in standards of care reduced Sustainable –Money invested in primary/community care NHS care into the community

How will it work? £480k investment every year in home-based treatment and community services £5.86M investment in ‘Centre of excellence’ in Little Hulton for very specialist care of older people with complex needs Extended working hours (9am-8pm Mon-Fri, and 9am-5pm at weekend) for community mental health teams Intensive visits to patients at home up to 3xday HOME public consultation: 6 th January 2014 – 30 th March 2014 –
