Risk Assessment HAZARD RISK/CONSEQUENCE RISK RATING PERSON AT RISK CONTROL MEASURES Portable Oxygen Cylinder Fire, Explosion, projectile objects, serious personal injury Medium Staff, Patients Members of the public Portable Oxygen Cylinder to be stored in a upright, secure setting where there is no risk of tipping or crashing to the ground. Oxygen tanks are used by applying a regulator, which allows for the release of the gas at a controlled rate. Caution must be applied when changing this regulator (dangerous projectile objects). Empty tanks must be disposed of as per manufactures instructions. No Smoking around any oxygen source. Needle Stick Boxes/Sharps Box Needle stick injury, Disease, HIV, HEP C, HEP B, High Patients, Never over fill a Sharps box. Wear PPE gear at all times. Always wear 2 pairs of gloves, and gown when dealing with blood/needles. Always have sharps box to hand when dealing with needles to avoid walking with needle to locate the box. Spillages Contamination, Falling, Minor personal injury Nursing equipment and materials, tools and small surfaces are cleaned with 70% alcohol. Large surfaces such as floors are disinfected with a chlorine solution 10000 ppm. Always wear PPE gear (Gloves) when cleaning spillages containing bodily fluid. Seal off the area after cleaning and toughly disinfect. Cables and sockets Slips, Trips, Falls, Minor Personal Injury, Low Staff Ensure no cables or leads are trawling along the floor. Plugs should be checked to ensure that they are correctly fused for the appliance that connects to it, i.e. desk lamp-3amps, electrical heaters-13 amps, VDU’s- 13 amps. Plug casings should be regularly checked and replaced if defective. Sockets should not be overloaded.
