Contextual conditions An aging population in Wiltshire. Within our over 65s age group, there will be a particular increase in the number of Wiltshire residents aged >75 (+13,086), and >85 by Care package budget constraints. 66% are self funded but personal budgets are stretched Area includes three Super Outputs Areas in 20% most deprived in Wilts New CCG funded Care coordinators working with the GP practices Social isolation and loneliness is a key concern for older people enquires by GNC service 2012/13. Social isolation and loneliness impacts on physical health and mental well-being of older people and that this is why it is important to address this key factor in helping to improve health and keep people out of hospital. In addition supporting the maintenance of social networks enables people to find greater support among friends/family at critical time e.g. on discharge from hospital Nationally, over a million older people go a month or more without seeing or speaking to anyone Not enough locally based provision which older people have support to access Identified lack of Non emergency passenger transport services and supported transport 90% of Wiltshire is rural Nearly two-thirds of people admitted to hospital are over 65. Unplanned admissions for people over 65 account for nearly 70% of hospital emergency bed days. From the Older People JSA: The numbers of people aged 65 or over suffering from depression is expected to rise by around 15% from 8,300 to 9,600. Patients with a single long-term condition cost about £3,000 per year whilst those with three or more conditions cost nearly £8,000 per year. Older people are more likely to need health and care services and we know that nearly half of Wiltshire’s NHS financial resources (47.4%) are used by people aged over 65. Key policy conditions CCG vision is that Health and Social Care services in Wiltshire should support and sustain independent healthy living Rate of falls admissions to hospital in over 65 per 100,000 population Wiltshire: 1936 (England- 2011)- not significantly different from England average CCG Five Year Strategic Plan aspires to create a model within which, when care is needed it can be delivered closer to home complimenting the services of health and social care with resources in the community and voluntary sector. Over 65s consume the largest volume of resources per head, as well as the largest overall proportion of the CCGs resources Dementia is identified as a priority. One in 14 people over 65 have a form of dementia Multi agency “Campus” developments across Wiltshire being developed Key priority is increasing the proportion of older people living independently at home following discharge from hospital. Programme Objectives Increasing routes out of loneliness Evidencing the benefits of early intervention and prevention Cost efficiencies Rationale Resources are stretched and we know that nearly half of Wiltshire’s NHS financial resources (47.4%) are used by people aged over 65. Looking forward, the financial future is characterised by austerity, with current national forecasts pointing to a £30billion gap in funding by 2020/21, despite NHS budgets being protected. The biggest factor that creates this pressure is from demographic change, especially increases in the elderly population. The savings of intervening early to prevent ill health can be made and better understood Inputs per year £30,000 for GNC/CO (Area Board, Trusts, CCG/WC?) £5,000 Community Transport Grants (TBC) Partnership training on safeguarding, befriending, CO/healthwatch Wiltshire Healthwatch Wiltshire volunteers 100 £13.90 = £1390 Actvities Good Neighbour Community Organiser is supporting at least 45 older people a year and social isolation solutions found 20 volunteers identified, recruited and supported as researchers, co- designers and project volunteers by partners to support routes out of loneliness each year Over 100 people reached through CO “Listenings” and other engagement exercises each year and shared with decision makers to influence policy and services New opportunities out of loneliness are developed 3 each year Partnership training programme delivered for volunteers including safeguarding Partnership training/workshops for referral partners including Care Coordinators New transport schemes helping 40 older people access support Promotion of services through local networks DCAP online newsletters, local events to professionals, clients and referrers Annual celebration event to recognise volunteers supporting loneliness agenda in DCAP area Ongoing mapping to improve signposting Outputs 20 volunteers trained 45 older people supported into social activity over 3 years 3 new community projects 36 people who are supported feel they have a say in developing their support package 40 people benefit from Community transport solutions Decision makers are better informed of local needs and gaps
