SDPI Diabetes Prevention Program The Special Diabetes Program for Indians Initiatives Coordinating Center supports the activities of two programs funded by the Indian Health Service Division of Diabetes Treatment and Prevention. Diabetes Prevention Program and Healthy Heart. The programs are conducted at 68 sites across the United States. SDPI Diabetes Prevention Program

SWO Diabetes Prevention Program Targets individuals at high risk for developing type 2 diabetes and focuses on lifestyle changes through education and community activities. SWO Diabetes Prevention Program

SWO Diabetes Prevention Program Has a 16 week curriculum Program Which intergrades the healthy eating habits and exercise Incentives each week Motivational speakers Healthy food preparation classes SWO Diabetes Prevention Program

Objectives To identify individual with pre-diabetes To screen and recruit individuals with pre-diabetes Complete and document DPP assessment and questionnaire Provide after-core activities on a quarterly basis for the 48 participants Provide less intensive community/group activities Objectives

Provide intensive diabetes education intervention for individuals with pre- diabetes in the DPP Provide opportunity for participants to exercise a minimum of 150 minutes per week by the end of fiscal year Provide guidance and opportunity for a minimum of 48 participants to successfully maintain 1200-1800 cals/day by the end of the 16 week intensive activities Cont. Objectives

Provide guidance and opportunity for minimum of 48 participants to successfully consume <25% of calories from fat by the end of the 16 week intensive activities. Provide guidance and opportunity for a minimum of 48 participants to successfully have a 7% weight loss and maintenance by the end of the 16 week intensive activities and the end of the fiscal year Retain 100% of Participants Cont.
