Goal: 48/year Screening at monthly Just Move It-Oneida events Pre-diabetes diagnosis – Not eligible: pregnant women, active alcohol/substance abuse, End Stage Renal Disease on dialysis, cancer OCHC Medical Referral Assessment/Data Collection Readiness Assessment Wellness Vision

7 Assessments Medical clearance
Clinical measures (weight, height, waist circumference, and blood pressure) Lab tests (lipid profile, glycemic measurement – A1C, urine albumin:creatinine ratio) Clinical history Prescribed medications

8 Diabetes Prevention Program Curriculum
16-week sessions with emphasis on: Self monitoring techniques Problem solving Individualizing programs Empowerment and social support Frequent contact with coach and DPP support staff

10 Individual Coaching Sessions
Wellness Vision/Baseline Assessment Meet biweekly or monthly during curriculum and quarterly thereafter Assessment of patient needs, goals, problem solving skills Individualized nutrition and/or physical activity as needed Motivational support

11 Retention Personal Training Incentive Plan
Points system for participation in all components of the program Points value: For every 25 points = $5.00 Oneida Retail Card
