Learn how to earn healthy rewards every step of the way “It pays to know your score!”

Why should I become an Activated Consumer? Individuals are more likely to make good decisions and take more action to promote their own health if they: Are more engaged Are more informed Feel confident that they can take care of themselves1 Individuals with a chronic disease who have the skill and confidence to manage their health are better able to function and experience fewer health crises and functional declines.2 When individuals are educated, engaged and feel confident in their ability to manage their health, they will experience better health outcomes and lower utilization of health are services. Individuals with chronic disease make many choices in their day-to-day lives that have major implications on their health and their need for care. They often must follow complex treatment regimens, monitor their conditions, make lifestyle changes, and make decisions about when they need to seek professional care and when they can handle a problem on their own. Those who have the skills and confidence to take on these challenges are better able to function and experience fewer health crises and functional declines. Health Action Step Two Complete Personal Health Profile Let’s take a closer look at the key features of Core+ - beginning with biometric screening. Medica has partnered with a nationwide vendor, US Wellness, to bring administrative ease and flexibility when collecting and providing biometric screening data. We offer both online and telephonic registration for completion of the screenings. A biometric screening is one way to assess risk for heart disease, diabetes and high blood pressure. Through our partnership with US Wellness, we offer biometric screenings in a workplace setting or through an at-home kit screening option. Trained health professionals manage the worksite screenings, collecting information to determine an individual’s level of risk. In addition to measurements for height, weight and blood pressure, tests for cholesterol, Triglycerides and Glucose are also administered. A home screening kit is also an option. Materials are mailed directly to the member’s home for a self administered screening. Materials are mailed to US Wellness in a postage paid mailer with results available in 2-3 weeks. A nurse will even call the member to review and discuss their results. The cost of the biometric screening is $50 for both the worksite and the at-home option. This cost is passed through as a preventive claim. Employers can choose to offer biometric screenings to non Medica members and will be billed separately for this service.
