Care plan: Who should write it, What does it contain and why it is important? Do you know any thing about care plan ? must be written on the day of admission by the physician Discharge plan: Who should write it,what does it contain, why it is important? must be written on the day of admission by the physician. It contain the plan to facilitate on time

Medication reconciliation : Who should write it ?what does it contain? Why it is important? medicine will be documented by the nurse and reconcile by physicians. It contain all the medication that the patient received. To reduce medication error. Progress Note : It should be date, time, pager number, and signature. Discharge Instruction: It is on the day of the discharge, reporting critical test inform the attending physician immediately/ team member

:preparing patient for procedure,rad,lab,pre-op preparation and any other procedure. Consent procedure: Blood transfusion Patient Education: in progress note. Code RED: What is Code Red, When It is Announced, What do you do? Code red there is fire or smock detected. Call 953. The Individual will apply: RACE PASS 8

Code White: What is Code White, When It is Announced, What do you do? It is announced when there is violence or disrupt behavior. Call the security 953. and try to be calm and keep your self away that person. 9

Code Orange : What is Code Orange, When It is Announced, What do you do? It is announced receiving threat for bomb by call or direct threat or on identification of suspicious box. Call the security 953. Tell the details. Evacuate your self or locked your self. Keep calm and instruct everyone to evacuate on the instruction of the security. 10

Code Grey: What is Code Grey, When It is Announced, What do you do? It is announced when there is utility failure, shutdown of electricity,O2 shutdown, communication shutdown, water failure. Call the security 953. Maintain patient care management and safety. Evacuate patients to other facilities,if appropriate. 11

Code Black: What is Code Black, When It is Announced, What do you do? It is announced when there is External disaster. The command centre will contact everyone to come to their duty. Call the security 953. and try to be calm and keep your self away that person. Everyone will follow his/her job action card. The card ill be available in the assigned area/in the command centre. 12

Environmental Check Check the following while you are in the department: Fire e extinguishers. Fire exits. Fire and papers in the offices. Infection control. Cleanness of the location,dept or offices. Ensure there are no hazardous materials around. es 13

Environment check Ensure electricity is well set. Ensure all equipment are tagged and labeled. Ensure there are no expired food. Medication refrigerators are checked for temperarure and food. Check sharp containers. Check ID badges. 14
