National Patient Safety Goals: PASS OR FAIL! NPSG – Patient ID- TWO identifiers Name and DOB ESPECIALLY FOR MEDS AND BLOOD TREATMENT!!!

NPSG – - Elimination of Transfusion Errors 2-person verification process to ensure match of blood/blood component to order 2-person verification to match patient to blood/blood component

NPSG – Improve Staff Communication Get important test results to the right staff person ON TIME Our policy? Results reported to the physician within one hour of result.

NPSG Label medications & solutions on and off the sterile field NPSG Reduce the likelihood of patient harm associated with anticoagulant therapy

NPSG Maintain and communicate accurate patient medication information Obtain list of patient’s home meds upon admission Compare home list to those ordered in the hospital Provide pt. with list of meds to take once discharged Explain meds to patient upon discharge

NPSG Reduce harm associated with clinical alarm systems Alarm Safety as a hospital priority Policies/Procedures for appropriate alarm settings, when parameters can be changed, when signals can be disabled, w Policies/Procedures for WHO has the authority to set or change alarm parameters Monitoring of RESPONSE to alarm signals Education of staff and attending physicians re:purpose and proper operation of alarm systems

NPSG Handwashing! NPSG Prevent Healthcare- Associated infections due to Multi Drug Resistant Organisms NPSG Prevent infection of the blood from central lines NPSG Prevent Surgical Site Infection (SSI) NPSG Prevent indwelling catheter-associated urinary tract infections (CAUTI)

10 What do residents need to know??
NPSG – Identification of patients at risk for suicide UP – UP (UNIVERSAL PROTOCOL) Prevention of wrong site, wrong procedure, wrong person surgery Verification process – do we have the right person? Site marking – do we have the right site? “Time Out”- Final, active communication prior to start of procedure Most troubling: BEDSIDE PROCEDURES!
