Berkshire West ICS Working collaboratively to deliver high quality and sustainable healthcare services for the local population Primary care alliances Our GPs across Berkshire West Through these organisations, the footprint of the Integrated Care System covers a registered population of 528,000 residents, living in three Local Authority areas: West Berkshire Council; Reading Borough Council; Wokingham Borough Council

Key objectives for Berkshire West ICS Improving patient experience and outcomes Financial sustainability across the ICS Improvement in the health and wellbeing of our population Berkshire West ICS objectives Urgent & Emergency Care Pathways Primary care resilience Future enablers Finance Five priorities Making faster progress in transforming the way care is delivered, as set out in the NHS Five Year Forward View, and in particular making tangible progress in urgent and emergency care reform, strengthening general practice and improving mental health and cancer services. Managing these and other improvements within a shared financial control total and to deliver the system-wide efficiencies necessary to manage the local NHS budget. Integrating services and funding, operating as an integrated health system, to manage the health of the local population, keeping people healthier for longer and reducing avoidable demand for healthcare services. Demonstrating what can be achieved with strong local leadership and increased freedoms and flexibilities, and share learning with the wider NHS.

Integrating care in Berkshire West

Achievements - one year on Integrated pain and spinal service – community based multi-disciplinary assessments and treatments for patients with persistent pain Redesigning Clinical Pathways: Outpatient appointments Urgent Care 111 Connected Care Programme launched – a shared health and care record for all patients across Berkshire West Identified Population Health Management as a key workstream Strengthened working between the ICS and the 3 local authorities Shared priorities Design our neighbourhoods Joint commissioning Established 4 Primary Care Alliances More resilient primary care Some services delivered by practices working together Part of the leadership of the ICS New ways of managing the Berkshire West £

What we have already heard At a previous engagement event delegates told us: We need to describe a common storyboard for the ICS Develop simple and consistent messages to share more widely Repeat ICS engagement events to keep stakeholders updated Making faster progress in transforming the way care is delivered, as set out in the NHS Five Year Forward View, and in particular making tangible progress in urgent and emergency care reform, strengthening general practice and improving mental health and cancer services. Aim of workshop To develop a blue print for patient and public involvement building on what we are hearing from our stakeholders Following this session and future engagement events we hope to have: Locally-owned and co-produced action plans for public engagement in our integrated care system A broader picture of ‘what good looks like’ and the engagement impact taking into account learning from the workshops in other ICS areas A stronger national network of ICS public engagement leads and practitioners
