Introducing … 1000 Lives Plus 1000 Lives Plus aims to improve patient safety and reduce avoidable harm, waste and variation across NHS Wales. 1000 Lives Plus - Making patient safety a priority

– why it’s still important Patient safety – why it’s still important More than one in ten people admitted to hospital are harmed unintentionally by its care. There is a one in 300 chance of accidental death through errors in care. 1000 Lives Plus - Making patient safety a priority

Moving care from the board to the ward to home 1000 Lives Plus is committed to accelerating the pace of change with an urgency to spread the new ways of working so that they become best practice everywhere – it’s what our patients have the right to expect. 1000 Lives Plus - Making patient safety a priority

A national programme 1000 Lives Plus will align with several other national programmes in the five year Strategic Framework for NHS Wales It draws together existing work from the 1000 Lives Campaign and Intelligent Targets. 1000 Lives Plus - Making patient safety a priority

Working together 1000 Lives Plus includes areas in primary, secondary and community care, and provides opportunities for different teams across Health Boards and Trusts to be involved in improvement. 1000 Lives Plus - Making patient safety a priority

Embedding best practice “1000 Lives Plus is designed to ensure that the new ways of working are embedded in everyday practice, enabling everyone to meet the shared vision of putting patients and patient safety above all else.” Dr Tony Jewell, Chief Medical Officer for Wales 1000 Lives Plus - Making patient safety a priority

Supporting organisations 1000 Lives Plus will support organisations by: Providing tools, techniques and interventions to reduce local harm and mortality Delivering mini collaboratives for each new programme area – which will be phased in over the next twelve months 1000 Lives Plus aims to provide organisations with the tools, techniques and interventions to support the achievement of local harm and mortality reduction. To support organisations in implementing new areas of work introduced as part of the 5 year programme, 1000 Lives Plus will deliver mini-collaboratives for each new area. We are conscious that Health Boards and Trusts will need to manage the resulting work load and therefore the mini-collaboratives will be phased (then go into details on different programmes?) 1000 Lives Plus - Making patient safety a priority
