Setting the scene: 1000 Lives Campaign
Welcome: Its the first 1000 Lives Plus national programme Board meeting and it is great to see so many people here.
If we can go around the table, please introduce yourself...
Before we introduce 1000 Lives Plus, I’d like to share with you the foundations for this work – the 1000 Lives Campaign.

I’m delighted to be able to update you on the work that has been taking place to improve both the quality and safety of the healthcare that’s being delivered in Wales over the last few years. This work has been focussed on the 1000 Lives Campaign – and the new 1000 Lives Plus programme which has succeeded it.
I’d like to take you back two years to the start of the 1000 Lives Campaign ...
An all Wales focus – engaging all of secondary and primary care, as well as the Ambulance Service
Six key content areas – clearly defined areas where progress could be made – recognising that no real progress could be made without the involvement of senior leadership and then specific clinical areas
Two main aims – to save an additional 1000 lives and prevent 50,000 cases of harm over a two year period.
Equally important, is the Campaign’s main aim to introduce and embed new ways of working that will be make the NHS in Wales safer not only for two years but decades to come. 3

4. Two years that made a difference
The Campaign is estimating that 1,199 additional lives have been saved by NHS staff in Wales between April 2008 and April 2010.
The final figures indicate 50,000 episodes of harm have been prevented, but it is recognised that the methodology needs further work in order to provide a greater degree of confidence.
The Campaign ended last April, and the latest figures were published at the end of November. 4 4

Over two years – we have seen real change within NHS Wales.
Here are just some of the main headlines of the work:
New networks have been established around key clinical areas
The commitment of frontline staff has been exceptional – “This work reminds we I entered the profession.”
Teams are now measuring the effectiveness of their interventions – assessing what’s work and modifying to bring further improvement.
Patient safety is now on the agenda of every Health Board and Trust in Wales. 5 5

6. Achievements
WHO Surgical Checklist used in operating theatres throughout Wales
Care bundles reduce number of ventilator associated pneumonias on intensive care wards
Action to reduce pressure ulcers, with one pilot ward reporting no ulcers for over 700 days.
Working with community pharmacies to ensure safe dispensing of Warfarin
Development of local pathways of care for heart failure services to enable a seamless patient journey.
Over 600 Safety WalkRounds - including 150 in community settings.
Clinical improvements
The World Health Organization (WHO) is now being used in operating theatres throughout Wales. It asks standard routine questions to reduce number of errors in surgery. Is this the correct patient, are we operating on the correct site, does this patient have any allergies to the anaesthetic
Care bundles – groups of interventions that when carried out together result in better outcomes for the patient, than when implemented individually.
Pressure ulcers – cost the NHS £2.4 billion per year, 4% of the UK’s NHS budget. SKIN bundle is now being introduced to organisations throughout NHS Wales.
WalkRounds – senior staff out meeting frontline staff – linking the boardroom to the frontline. 6 6
