Our Treatment Plan Healthcare Domain: Primary Care Provider: Physician Objective: Decrease symptoms as indicated by client report of: (a) improved school performance (b) improved focus on task (c) decreased hyperactivity (d) improvement in behavior/mood both in the home and at school   Goals: a)  Connect to community based resources for both diagnosis and ongoing therapy as needed b) Identify etiology of symptoms in order to develop treatment plan c)  Outline treatment plan Interventions: a)  individual/family counseling b)  medication c)  other community or online resources d) consistent follow up

Measuring Outcomes/Success a) individual/family counseling b) medication c) other community or online resources d) consistent follow up

Communication Needed from Counselors Your assessment of any identified comorbid disease Your feedback when sx are severe enough to consider medical management Compliance or noncompliance with treatment plan 4. Communication with primary care can occur via a written consult report or in more emergent situations, by phone.
