Presentation transcript:

John L. Sapp et al. JACEP 2017;3:687-699 Localization of Ventricular Tachycardia Patient #1 (A) Monomorphic ventricular tachycardia (VT) captured by the handheld device for Patient #1. The 12-lead electrocardiogram with root mean square function (RMS), detection function (DEL), and time scale (seconds). Onset of 1 VT cycle is marked at 248 ms from the beginning of the recording. (B) Screenshot from the handheld device for Patient #1. Display with the bull’s-eye projection and the opened ventricle. A number within each segment is correlation coefficient (CC) for template matching of VT pattern with each segment’s template. Template matching identified segment #16, apical anterolateral (marked by the thick white frame), with a CC of 99%, as containing a VT origin; the regression method put VT origin into triangle #2 on the edge of the neighboring segment #15. (C) The 3-dimensional electroanatomic substrate for Patient #1 is shown, left, with an inferior view of the left ventricular endocardial map, apex at top, mitral valve at bottom. Areas with bipolar signal amplitude ≥1.5 mV are indicated in purple. Right is a mesh map of the same chamber with the predicted site of VT exit noted (purple dot). The site of VT exit identified (during VT) by activation and entrainment mapping is indicated in both images by the yellow arrow. John L. Sapp et al.
