Isochronal Analysis of Right and Left Ventricles During VF (A) Isochronal analysis of RV and LV during VF in a 73-year-old patient; ejection fraction, 25%; presenting for drug-refractory VT. LV isochrones show a rotor (CL, 220 ms) in the septal LV. This rotor persisted for 15 continuous rotations (shown ≈5 s into VF). Rotor activity was seen in 72% of all VF cycles in this patient. (B) Basket EGMs during VF near the rotor core are numbered 1 to 6. Note that activation spans >80% of the VF CL. Artifact before cycles 3 and 4 of EGM 3 is noise (absent on the other EGMs). (C) Wave front vector analysis of the subsequent VF cycle shows consistent rotation about the core with radial activation of distant tissue. Reprinted with permission from Krummen et al. (11).
