Termly Pupil Tracking Meeting Appendix 3 Confirm Quality inclusive differentiated Wave 1 Provision and whether there is a need to implement a Wave 2 Intervention Agree Wave 2 Intervention, support and expected outcomes and record on school system, monitor progress for half a term If no progress has been made then assessment tools are used to inform all stakeholders to see if a barrier to learning can be identified (Quality Wave 1 and 2 to continue throughout this process and monitored termly) The Plan-Do-Review process involving all stakeholders is initiated and ILP (Individual Learning Plan) is written and put in place, which is then reviewed termly Determine whether the ILP needs revising and if pupil needs to remain in the SEN register or the cycle repeats If a barrier to learning is identified then the pupil is placed on the SEN register and Wave 3 provision is planned Termly Pupil Tracking Meeting Pupil is not making expected progress Review to assess overall impact of the ILP SEN Identification Graduated approach
