VirtualHealth Implementation for OBCM and CC4C

VirtualHealth Rollout VirtualHealth go-live is October 29! CMIS will “go dark” on October 25 but will be available for several months in a read-only format Many patient files and associated documents will be migrated to VH There are no major changes to the OBCM and CC4C programs themselves Documentation workflow will change Patient care will not change

VirtualHealth Benefits More automation, less care manager time spent on manual documentation processes Care-plan driven approach with decision support Improved efficiency of system function Examples: Care plan populates needs/problems/goals/ interventions based on care manager documentation in comprehensive assessment Ability to work in multiple windows at the same time Ability to save assessment documents as drafts Automated task when a new referral is received

Project Summary to Date Project Status is officially out of Red and in Yellow status 17 Work Packages have been completed All Day 1 Work Packages are either completed or in flight First Mock Data Migration has been completed Completing end to end review of VH workflow Training project plan finalized Core Trainers selected Monthly webinars and FAQ documents disseminated User Satisfaction baseline survey completed Prioritized post-Day 1 Work Packages to continue without loss of momentum
