6 NCServes: Building Tangible Impact
Our Impact Statement: “Better Together” Our method to delivering impact: the community being on one shared platform to coordinate & better deliver services in health, education, and employment. How do we coordinate care? CC Goodwill decides that mental health services would be better provided externally. Goodwill creates & submits referral in the platform for the client. VetCenter Jacksonville receives, reviews, and accepts the referral if it is a correct fit. Coordination Center (run locally by USO of NC, Charlotte Bridge Home, ECHSA, and ABBCM) receives referral and finds the right provider for mental health services. Veteran is at Goodwill for employment help. Also is in dire need of mental health support. Veteran receives counseling services by VetCenter Jacksonville.
