Sunderland Young Carers Pathfinder

Pathfinder – What Is It? Young Carers Pathfinder / Family Pathfinder – Two separate entities linked by common themes. Pathfinder is not seen as a new area of work. Mainstreamed through the Multi-Agency Partnership Board.

Aims The early identification of young carers and preventing them undertaking inappropriate caring roles. How will we do that? Develop a system whereby clinicians will identify a potential young carer at the point of diagnosis of a long term condition or illness of a related adult. A practitioner’s guide for working with young carers has been developed. Clear guidance & protocols on actions & responsibilities will be developed for neurology departments and psychiatric services.

Aims Intensive action with a group of families to lift children out of inappropriate caring roles. How will we do that? Annual family fun day consultation events. Targeted families. Advocacy to engage with mainstream services. Not all families will engage with statutory services. Support packages provided through pathfinder, tailored to individual family needs.

Aims To identify “hidden” young carers, enabling them to obtain the support they may need. How will we do that? Work closely with education to provide them with the tools to identify & support young carers. Promote the use of the Young Carers School Card. Increase the visibility & circulation of the Young Carers Newsletter. Promote the Young Carers Peer Support Line. Develop and distribute Young Carers Packs. Strengthen our links with key partners.
