2. Where will Team around the Family be targeted in Cardiff?

3. Whole Cardiff model Referral into the TAF system.
Phase 1 - Whole Family engagement and assessment, with all key members of the family using agreed JAFF structure. (Week 0-6)
Phase 2 - Team around the family planning meeting with the family and key agencies convened by the lead professional, family plan is drawn up and agreed based on the assessment above. (Week 7)
Coordinated multi agency intervention takes place in partnership with the family
Mid point review with the family, option to continue or adjust the plan. (Week 14)
2nd half of the intervention undertaken
Final review
Maintenance Plan agreed with the family (Week 20)

Unmet needs identified with the family, short period of additional work to be agreed and back to final review

5. TAF Manager post (1) Overall management of the Team around the Family.
Direct Line management of Senior Family Case Workers.
Strategic coordination and development of Team around the Family – working closely with the Central Families First Team and Partnership

9. Strategic link to Neighbourhood Management in Cardiff, Children Services, and other key structures such as IFST and Flysing Start.
10. Report to and work with the Senior Management Team supporting the Team around the Family process in Cardiff.
11. Represent Cardiff and contribute to Team around the Family Regional and National Learning Groups ensuring that learning and best practice is fed back in to Cardiff processes.
12. Work towards trainer status in Restorative Approaches in order to provide management training and consultation

7. Senior Family Case Worker (Part Practitioner)
Half case load.
Supervise family case workers.
Manage team case load and prioritisation.
Provide consultation and deliver training on Team around the Family processes.
Work towards trainer status in approaches that underpin the Team around the Family process, e.g. Restorative Approaches
Lead Professional attached to identified neighbourhood management groups in areas of high need and potential high referral.
Key link worker to partners in Children Services where families are being referred to and from a higher level of need.
8. Support the effective use and development of the JAFF

8. Family Case Worker (Practitioner)
1. Engage Families using Restorative Approaches/Integrated Family Support approaches
2. Undertake a whole family focused restorative assessment including one to one with families and whole family group meetings.
3. Engage and consult with referrers and key professionals, identified to support the family, in one to one discussions and in Team around the Family multi agency meetings.
4. Coordinate the Team around the Family process in partnership with the family.
5. Coordinate Peer Support opportunities for other professionals to underpin Team around the Family.
6. Lead Professional attached to identified neighbourhood management group and other key structures in Cardiff.
7. Support the effective use and development of the JAFF

10. Families First Team around the Family Neighbourhood role
Locality based Team around the Family workers could provide additional resource for neighbourhood management teams and enhance their multi-agency problem-solving role. They would compliment other neighbourhood based staff:
