Note: Entries must be written on the front page only. Pages should be numbered in the upper right corner. All entries must be made in blue or black ink. Do not use white-out if you make a mistake, simply cross out the error.

Investigative Question: State the scientific question you are investigating. Background Research: Include background information, definitions, and research you did during the course of your project. Use a new page for each individual source (5 sources are required). Resources Cited: List all sources of information using the proper APA format. For every source write the following information on the page before you begin taking notes: Author Date of publication Title(s) Book Article title and magazine title Website title Publication information City: Publisher Volume number, issue number Sponsor of website URL address for on-line sources Page numbers for print sources If you copy information from product labels, credit the manufacturer and label.

Note: If you use animals in your research, you MUST have research notes on their care. If you use chemicals, you MUST have research notes on safe handling and proper disposal. If you use bacteria or fungi, you MUST have research notes on safe handling and disposal.

Purpose: Your reason for doing this experiment. Hypothesis: State your hypothesis about the proposed relationship between the test (independent) and outcome (dependent) variables. Materials list: Complete list of materials used. Procedures: Write a complete, numbered, step-by-step description of how you conducted your investigation. Data: Try your experiment at least 3 times. Record all details of your experiment in your notebook including steps, trials, observations, thoughts, problems you encounter, changes made, or errors. Organize the numerical data into table(s). Results: Find averages of all measured data. Display your results on a graph. Analysis - This is where you explain your results.

Conclusion: Write the conclusion to your investigation Conclusion: Write the conclusion to your investigation. Restate the investigative question. Was the hypothesis supported/ not supported? Use results to support the conclusion. Include error analysis and what was learned. Acknowledgments: Give credit to anyone who gave you any special advice or help.
