AAC Monthly Report RA-UG Mine September 2016

Safety Performance With an understanding that all incidents are preventable, and all tasks can be completed without harm, extra focus, training and improved supervision must occur to reduce the Frequency of incidents. September Safety performance quite positive month without any recorded LTI. First Aid case reported when a workman was lifting the pipe and putting in a scissor lift with the help of two other workmen. His hand struck between scissor lift platform and pipe sustain cut on the same.

Safety Positives Action Plan Continuing delivery of SOPs to all employees.( role specific SOP completion have been targeted to ensure coverage of specific requirements) To complete SOP coverage for all new employees before release into the work teams. Safety Tools box talks and campaigns on:- Housekeeping, Checking misfired holes, Working around open holes, Life Saving Rules, Refuge chamber use, Reversing of mobile equipment and Handling trailing cables. 4. Safety tool box talks are being delivered to all employees at start of every each shift. Reviewing, briefing and enforcement of all SOPs. Ensuring that supervision is adequate for all employees performing tasks.

Performance Review Production Drilling September was an another positive month for development. Overall mine performance was 1399.63 meters. AAC performance 765.68 meters. RDC performance 633.95 meters. Production Drilling Production Drilling for this month is 6060.2 meters.

SOLO Performance Total All In Meters for September = 7561 mtrs Availability vs. Utilization combined was 73% vs. 29.5% (Excessive back reaming, bogged Rods as Percussion not used) Total combined Power Pack Hours = 308.2hrs Total combined Percussion Hours = 127.9hrs Drilling Utilization = 60% Completion Rate from Designed Meters was 93.42% Combined Percussion Rates for Month at 59.3Mtr/per/hr 675mtrs achieved in 17hrs of Drilling time.

Graph Indicators This month AAC has achieved 114.28% of our target. This month hauling higher than pervious month tonnes. Production Drilling was below its target. Over all 2367 cu. Meter shotCrete material sprayed.

Improvement Opportunities and Special Focus Areas AAC and HZL management teams focusing on improving the utilization of equipment , downtime and availability. AAC trainer to continue developing national operators into a multi skilled workforce, mitigating operational impacts. Continued QA/QC management between HEMM, OEM’s and AAC regarding maintenance scheduling, damages and downtime impacts against operations. SOP completion will be monitored and implemented as opportunities arise in between operational needs and HZL safety pillar training sessions. Maximising the use of LF600 agitators for delivery of shotcrete (increased capacity).

Improvements Tracking of Jumbo plods to identify impacts on utilisation of drills. Dedicated mining engineer tracking and analysing utilisation of every piece of equipment. Allocation of operators to specific rigs to improve utilization and availability of drill rigs. Identifying the best way to control unauthorised absenteeism. Hot seating of equipment during shift change over. Loader and shotcrete support for development targeted to reduce impacts. Mentoring/ coaching of our engineers to identify processes for best utilization of available resources.

Stand for Diesel Filling in CAT Trucks Problem Statement:- The height of diesel filling point in CAT truck are high. Solution :- To over come this problem new stand was fabricated on which person can stand and fill the diesel. Stand for Diesel Filling in CAT Trucks

Spanner Stand Problem Statement:- The spanner stored in a common box so it become difficult to find the right spanner easily. Solution :- New spanner stand was fabricated in which each spanner has its on place.

Rusted bolt thread cleaner Problem Statement:- Due to underground environment the bolt thread to much rusted. Solution :- To clean the thread of bolt new cleaner fabricated for bolt size 12,16,20,24,36mm. Rusted bolt thread cleaner

New Support for Paste Polly Pipe Problem Statement:- The support installation for paste line taking too much time and after installation vibration remain. Solution :- The new design of paste line support take less time and reduce vibration in line. New Support for Paste Polly Pipe

Training Training for the month was as follows: SOP Trainings:- 379 SOP’s were covered for personnel, Coupling and uncoupling of trailing cables, PPE, Drilling and Blasting etc. Mobile Equipment training & Authorization:- 16 authorizations completed for the month including PC, Scissor Lift, Mine Truck, Miller, LF600 Agitator, CAT AD60 Mine Truck. AECT Training:- Action Employees Can Take. Road safety awareness training.

SOP Training SOP training:- This included different types of SOP training i.e. Sop Use of Self Rescuer , Coupling and uncoupling of trailing cables, Job safety environmental analysis, Drilling and Blasting etc.

Mobile Equipment Training Mobile Equipment training & Authorization :-Total 16 employees were trained and authorized (including multiple authorizations for some personnel) on different mobile equipment: i.e. :- Cat AD 60, Mine Truck, LF600 Agitator, cable bolter, fork lift, jumbo, SOLO.
